Case Manager Assistant - Case Management
Responsible for assisting the Case Manager in the review and evaluation of the patient. Also responsible for participation in the interdisciplinary team and communicating effectively to achieve desired clinical and resource outcomes. Responsible for assisting with coordination of patient/family education as needed in relationship to discharge planning efforts. BLS required. Acute care or long term care experience is preferred.
